VENDOR APPLICATION FORM  


Dear Vendor: Please fill out all the required information as accurately as possible. Your submitted data will be reviewed by our Planning  Committee and you will be contacted by email within 10 days of your application. 

Please read the requirements of your participation and you  are invited to call or email with any questions you may have.  


Requirements for Participation as a vendor in the 2021 Seasoned Delicious Foods Sunday Market  @ The Evolution Center


1. Vendor must be a registered business entity: LLC, Corporation or NYS DBA. 


2. Vendors must have current insurance and name Seasoned Delicious Foods as the additionally insured. 


3. No application will be approved until full payment is received. Any cancellations once approved will not be refunded. The market is open RAIN OR SHINE.


4. All vendors are 100% responsible for cleanup at the end of the event.


5. Market days will be every Sunday starting May 16, 2021 through October 31, 2021 12:00PM ET - 5:00PM ET with one closure on Father’s Day June 20, 2021. 


5. Vendors will be granted access to the site at 10:00 am the morning of the event and must be completely set up by 12:00 pm. All vehicles  must be parked outside of the area after setup. There is no parking in the event parking lot. All vehicles can park in other areas of the park.  Break down is at 5:00 pm and vendors must be open until that time. There will be no allowance for early breakdown or departure. 


6. All vendors are responsible for attaining permits from their regulatory agency. Attach any applicable state and/or local licenses required for your business. Prepared food vendors and those selling taxable items must provide a copy of their NY State Sales Tax Authority certificate IN ADDITION TO ANY FOOD-PREPARATION LICENSES REQUIRED


7. Permits (copies to be attached to your application) are required for all prepared foods and foods served to eat in the market. It is your responsibility to be in compliance. We are inspected by the Ulster County Health Department. 


8. If you need access to electricity for which there will be a:

-  one-time seasonal charge of $100/per outlet/season 

or 

- $5/Day/per outlet

Please advise us so that we may take this into account in determining your space in the layout.


9. Vendors are expected to be civil and respectful to each other and to customers. The Farmers Market Committee reserves the right to dismiss a vendor for offensive behavior


10. Electrical Service (110-volt maximum)- 

Add $5 per day per outlet or $100 per outlet for the season


11. Registration fees:


Product Vendors: $35 per day or $700 for the season (save $140)


Food Vendors will be $50 per day or $1,000 for the season (save $200)

Important Notes:


10x10 space is provided to all outside vendors and a 10x10 pop-up tent may be used. Please be certain to provide adequate tent  anchoring to prevent damage from winds. All vendors can be provided with a 6-8 foot table and 2 folding chairs upon request (fee of $20).  

The following instructions are from “Interim Guidance for the Operation of Farmers’ Markets (NYS Dept of Ag & Mkts, updated 2020.08.03): 


Masks: “Face coverings must be worn at the market. Anyone who is over age two and able to medically tolerate a face covering must cover their nose and mouth with a mask or cloth face covering when in a public in accordance with Executive Orders 202.17 and 202.18, and any successor thereof.” 


Gloves: “Frequently wash hands with soap and water or use hand sanitizer containing at least 60% alcohol, if soap and water are not available. ▪ Gloves are recommended while handling products at the market.” 


Product display: “Minimize the food customers may directly access, and instead serve patrons.” “Do not permit customers to spend an excessive amount of time near the booth or table.” “Limit the number of customers permitted at the table at one time, allowing for proper social distancing.” “Set up display tables in a manner that allows for social distancing between consumers (e.g., do not create close or confined spaces with displays in a manner where social distancing cannot be maintained).” “Pre-package raw agricultural products, such as apples, potatoes, onions, etc., to the greatest extent possible. All other foods, such as breads and baked goods, must be sold pre-packaged.” “Do not allow customers to place personal objects, such as reusable bags, money, purses, or cell phones, on vendor tables.” 


Sanitation: “Frequently clean and sanitize surfaces and other frequently touched points of contact, including point of sale terminals.” 


2021 SFM COVID-19 Safety Measures-  If you need to, request pickup from your car for pre-paid orders only. This service will be provided by the market between 1-2pm only. “PICKUP ONLY” signs will be at designated parking places. To expedite your order, please bring a list of vendors from whom you ordered and give your name. A Market Assistant will come to your car to get that information and bring back the packages to your car.  Designate one shopper per family to be in the market  Wear a mask. This is required to keep all those around you safe. If you cannot wear a mask, please request a friend to shop for you or pre-order and pickup from your car.  Maintain a 6’ distance from others. See 6’ space markers on the ground.  Hand sanitizers will be available in the market  Note that displays will be simplified. One of each product for clear choice. The bulk of the product will be behind the vendor. Point, ask, and shout!  Do not pick up products. Vendors will assist you and bag products  If you can’t find what you are looking for, ask our friendly market staff/volunteers. No problem asking from 6’ away, as we have excellent hearing!.  Credit/debit cards for payment are preferred by many vendors; however, some do not accept them. The market provides Market Bucks for sale by credit/debit to use at those vendors who only accept cash.  Spend as little time in the market as possible.



FOOD VENDOR WAIVER OF LIABILITY EVOLUTION FARMERS MARKET 2021 


By checking the required box below, the person and the entity completing this form hereby agrees to indemnify and hold harmless the Evolution Center Farmers Market and their representatives, employees, and officers from and against any and all losses arising from or growing out of participation in the Evolution Center Farmers Market at 11 Lohmaier Lane Lake Katrine NY, whether it be caused by the negligence of myself, my agents, servants, employees, or otherwise. I understand that it is a contractual obligation for me to carry liability insurance as a condition of having a space at the Evolution Center Farmers Market. I agree to provide the Evolution Center Farmers Market with a Certificate of Liability Insurance evidencing coverage of no less than One Million Dollars ($1,000,000.00) per occurrence in Combined Single-Limit Bodily Injury, Property Damage insurance and Completed-Products liability, naming as Specifically Designated Additional Insureds all of the following:Seasoned Gives 11 Lohmaier Lane Lake Katrine, NY.
